Vaccination Vacation: Measles Makes a Menacing Comeback

In 2022, a concerning global surge in measles cases and deaths occurred, marked by a significant increase in outbreaks across 37 countries.. Centers for Disease Control and Prevention (CDC) attribute this rise to a decline in measles vaccination rates, particularly in low-income countries. Shockingly, over 33 million children missed measles vaccinations in 2022, exacerbating the situation and posing a serious threat to global health.

The CDC warns that the global measles threat is escalating, with millions of children remaining unvaccinated, creating an environment conducive to the spread of the disease. In comparison to 2021, measles cases increased by 18%, and deaths rose by 43% globally in 2022, totaling an estimated 9 million cases and 136,000 deaths, predominantly among children.

Widespread outbreaks affected 37 countries, with 28 in the WHO Region for Africa, six in the Eastern Mediterranean, two in Southeast Asia, and one in the European Region. Health officials express their concern, emphasizing that the increase in measles outbreaks and deaths was not unexpected given the declining vaccination rates witnessed in recent years.

While there was a modest increase in global vaccination coverage in 2022, with a first-dose coverage rate of 83% and a second-dose coverage rate of 74%, these figures fall well below the recommended 95% coverage necessary to protect communities from outbreaks. Low-income countries, where the risk of death from measles is highest, continue to have the lowest vaccination rates at only 66%, showing no recovery from the decline during the pandemic.

Of the 33 million children who missed measles vaccine doses in 2022, 22 million missed their first dose and an additional 11 million missed their second dose. The lack of recovery in measles vaccine coverage in low-income countries following the pandemic is a cause for alarm. More than half of the children who missed their first measles vaccine dose in 2022 reside in just 10 countries, including Angola, Brazil, the Democratic Republic of the Congo, Ethiopia, India, Indonesia, Madagascar, Nigeria, Pakistan, and the Philippines.

Health authorities stress the urgent need for improved vaccination coverage and response efforts globally to curb the spread of measles. CDC and WHO call for countries to locate and vaccinate all children against measles and other vaccine-preventable diseases. They also encourage global stakeholders to support countries in vaccinating their most vulnerable communities. Additionally, investing in robust surveillance systems and outbreak response capacity at all levels is deemed crucial to rapidly detect and respond to outbreaks.

To address this global health challenge, CDC and WHO are active members of the Measles & Rubella Partnership (M&RP), a global initiative committed to halting measles and rubella. Guided by the Measles and Rubella Strategic Framework 2030, the initiative aims to reverse the decline in national vaccination coverage, expedite COVID-19 pandemic recovery, and accelerate progress towards a world free of measles and rubella. The partnership includes other key entities such as the American Red Cross, Bill & Melinda Gates Foundation, Gavi, the Vaccine Alliance, United Nations Foundation, and UNICEF.

Ramadan 2023: Your rights at work

Ramadan 2023: Your rights at work

‘It’s important that UNISON’s Muslim members feel supported in the workplace and employers ensure and understand their needs at this spiritual and cherished time’

The holy Muslim month of Ramadan is due to begin on Wednesday 22 March, with the first fast from sunrise to sunset on Thursday 23 March.

Eid-al-Fitr, the celebration that marks the end of Ramadan, is expected to fall on Saturday 22 April.

During this important holy period for Muslims, UNISON reminds both workers and employers that it is within the Equality Act 2010 for all employers to ensure flexible working and provide reasonable adjustments for workers wishing to observe Ramadan, which includes fasting, prayers, charity, and reflecting on the Quran.

During Ramadan, Muslims spend a period of 30 days abstaining from food and drink, including water, during daylight hours, as a means of celebrating and reflecting on their faith.

Iftar, the meal that breaks the fast when the sun has set, is often shared with family and the local community. Traditionally, the fast is broken with a date (pictured above).

Advice includes:

  • consider the energy levels of staff, especially where work is physically demanding, and where possible, scheduling meetings earlier in the day and avoiding evening meetings or work events
  • avoid scheduling lunchtime events
  • be fair and considerate in the distribution of additional rest breaks
  • as the month of Ramadan is aligned with the lunar calendar, Eid-al-Fitr has no fixed date. Employers are encouraged to be flexible with their annual leave requests from Muslim workers around this time.

Guidance from ACAS and the EHRC provides employers with a range of best practices during the holy month, for example, by allowing Muslim staff longer on Fridays for Jummah prayers, by arriving or starting their working day earlier.

2021 survey of British Muslims on support from employers during Ramadan found that the most supportive adjustment (desired by 69% of recipients) was flexible shift patterns, followed by team members understanding what Ramadan means to them (60%), and annual leave for the final days of Ramadan (58%).

Wishing all UNISON members Ramadan Mubarak, UNISON national officer for racial equality Margaret Greer said: “Ramadan is a time of self-reflection, giving, and togetherness. For our Muslim members, this is an important time of the year to give thanks for the blessings and express gratitude in one’s life.

“It is important that UNISON Muslim members feel supported in the workplace and employers ensure and understand their needs at this spiritual and cherished time.”

A Good Diet for You

Biden admin expert claims obesity cannot be treated with exercise and a good diet

A physician who came under fire for comments claiming that obesity is more of genetics than lifestyle in a "60 Minutes" interview earlier this month is now a member of the Biden USDA's 2025 Dietary Guidelines Advisory Committee, which will help determine dietary standards for Americans over the coming years.

Dr. Fatima Cody Stanford, a doctor specializing in obesity medicine at Mass General Health in Boston, claims diet and exercise have little impact on the disease that affects nearly half of all Americans, so what does she think people should do about it?

"The number one cause of obesity is genetics," Stanford told CBS Lesley Stahl.

"That means that, if you were born to parents that have obesity, you have a 50 to 85% likelihood of having the disease yourself even with an optimal diet, exercise, sleep management, stress management," she added.

Stanford elaborated during the interview that an overwhelming majority of U.S. doctors are biased toward obese patients.

"Are you saying that doctors don't understand obesity? Doctors?" Stahl asked.

"Doctors do not understand obesity," Stanford confirmed.

Her comments coincide with an uptick in the disease in recent years, with the CDC reporting that the percentage of obese Americans shifted up to 41.9% in 2020 from 30.5% in 2017.

Even more concerning statistics from Harvard University's T.H. Chan School of Public Health indicate 43 million preschool-aged children were obese in 2010 worldwide and the numbers have been on a steady incline since the 1990s.

"If nothing is done to reverse the epidemic, more than 1 billion adults are projected to be obese by 2030," the page reads.

The Dietary Guidelines Advisory Committee consists of 20 "nationally-recognized" physicians, according to the USDA, and was announced by the agency's Secretary Tom Vilsack as well as the Department of Health and Human Services Secretary Xavier Becerra last Thursday.

"The Committee will be tasked with reviewing the current body of science on key nutrition topics and developing a scientific report that includes its independent assessment of the evidence and recommendations for HHS and USDA as they develop the Dietary Guidelines for Americans," the USDA's press release on the committee stated.

Further along, the report focuses on an equity lens to health, claiming that the committee will focus on socioeconomic status, race, and more to better determine how to approach obesity.

"The 2025 Dietary Guidelines Advisory Committee will examine the relationship between diet and health across all life stages and will use a health equity lens throughout its evidence review to ensure factors such as socioeconomic status, race, ethnicity, and culture are described and considered to the greatest extent possible based on the information provided in the scientific literature and data."

The committee updates federal dietary guidelines every five years, acting as a cornerstone in public health perception.

Diets for weight loss come down to one thing, and it's not technical language.

Diets for weight loss come down to one thing, and it's not technical language.

Peak diet season begins in January, so you're not alone if you're grabbing for the newest weight-loss plan. But how can you tell if that option is genuine or just another fad diet?

It is moving its lips.

What, then, is a crank diet? Here is my opinion: It's a tactic for eating less that's disguised as a technical justification for how the specific meal combinations being suggested work their metabolic magic. The book wouldn't sell many copies if the plan simply stated, "Eliminate high-carb foods and you'll end up eating less." Its lips begin to move as it realizes there must be a justification.

Let's be clear: A crank diet can definitely, positively help you lose weight. You are not alone if you have lost weight while following a diet that excludes fat, carbohydrates, gluten, plants, meat, or sugar. The truth is that you've only managed to lose weight if you've discovered a means to consume fewer calories than you burn off through exercise. One strategy for achieving this is to ban certain dietary groups. A beneficial method, at least temporarily, for many people.

Okay, so you might be wondering why I wrote an entire column about how cranky such diets are if they actually work.

Of course, truth, justice, and the American way. But perhaps empowerment as well. Because people should be able to recognize when they are being misled.

Let's examine a few entertaining instances of diets that adhere to the crank model:

According to the grain-free diet advocated in the movie "Wheat Belly," when wheat is digested, polypeptides have produced that bind to opioid receptors in the brain and increase desire.

Due to a "cascade of additional imbalances" of hormones linked to hunger and fat storage brought on by the insulin spikes caused by carbohydrates, the carnivore diet promises to reduce hormonal oscillations.

According to intermittent fasting, limiting your intake for a long time forces your body to use its fat reserves, which causes you to lose more weight than you would if your body had constant access to blood sugar.

According to the blood type diet, we thrive on the same foods as our ancestors since your blood type reveals your lineage. And there are other additional ancestral diets that reinforce this.

Of course, low-carb/keto also maintains that if you don't eat carbs, you don't produce insulin and you store less fat because insulin is essential for fat storage.

Fair enough, there are a few diets that outright state that all they are is a way to eat less. One of the main arguments for a low-fat diet is that because 1 gram of fat has 9 calories and 1 gram of protein or carbohydrate has 4, substituting lower-calorie macronutrients will result in fewer calories consumed overall. The Volumetrics diet also holds that eating less calorie-dense food will result in you eating fewer calories overall.

Despite the absurdity of certain diet justifications, not all of them are untrue. For instance, insulin actually promotes the storage of body fat. The only thing you truly need to know about food and health is that what we know is completely dwarfed by what we don't know. This is the one nutrition fact that stands above all the rest.

Do you recall the story about the blind men and the elephant? Six blind individuals were given the opportunity to "see" an elephant by becoming a part of it, and as a result, they all had quite different perceptions of what an elephant was. The person holding the tusk perceived it as a spear, whereas the person holding the trunk perceived it as a snake. You see what I mean. They couldn't feel everything, so they left with false impressions.

Diets are responsible for this. No one is able to view the entire elephant. It is not depicted by science (yet). Each diet expert so picks out a particular aspect of the human metabolism and declares it to be the secret to good health and weight loss when in reality it is just the toenail. Yes, the digestion of wheat results in polypeptides. However, the human body is undergoing so many other processes that it is difficult to predict how it would pan out.

Naturally, there is only one way to learn: via genuine trials. And, surprise, surprise, the ones we do have (and we have many) demonstrate that, over the long term, no diet is effective for losing weight. The pattern is the same for all subjects: they initially lose weight, sometimes for as long as two years, before gaining it back.

But let's get back to the bit about how crazy diets truly cause weight loss. How come? Because, if we cut through the science, there are usually some really good methods for implementing the fundamental weight loss strategy of eating less.

So, how about this: Ignore the technical details and get right to the point? Even while intermittent fasting does not perform better than other diets, this does not imply that it is a bad idea to lock the kitchen after dinner. It's actually a really smart idea.

Next, consider low-carb. Although insulin does not directly link to subsequent eating and weight gain, avoiding sugar and refined grains is still a good idea. It's actually a really smart idea.

You don't need to be an expert on human metabolism to lose weight because diet is not a scientific issue. Diet is an issue that you must solve; you simply need to come up with practical methods for eating less. Consider the deluge of fad diets as a smorgasbord of approaches and pick and choose the ones that will work for your way of life.

I adopted strategies from many diets to maintain my current weight loss after being overweight in the past. Although I don't practice intermittent fasting, I do lock the kitchen after dinner and wait until I am extremely hungry before eating. I don't consume a low-fat diet, but I do try to keep additional fats in my food to a minimum. I don't consume a lot of processed carbohydrates, but I'm not low-carb either. I add vegetables to recipes to make them more filling (Volumetrics). I hardly ever consume highly processed foods (every diet known to man). Common sense dictates that I don't leave foods that are convenient for me around the house, and when we have to purchase Girl Scout cookies for neighborhood peace, I have my husband hide them (well, nobody advises doing that, but it works for me because I'm thin).

Crank diets prey on individuals who want to change, often desperately. This is what I detest about them the most. All I have to do is this one thing! — the biochemical explanations offer a sliver of hope, and then the end failure feels like your failure. But anyone who has attempted it knows that losing weight is challenging. No one thing exists. And only you can determine where your eating habits veer off course, which foods are your downfall, and whether or not modifications work for you.

I salute those who are content with their weight and concentrate on other elements of their health. I'm not one of them, which is unfortunate because I hated being obese. Perhaps this is why the hype about diets that draws people in irritates me so much. But I also believe that, at least in theory, losing weight is not only feasible but also very simple.

Forget the polypeptides; it's not a known problem. It's a problem that needs to be solved, and only you know how.

Loading up on vitamin C? Too many supplements can be unhealthy, dietitian warns

Some Michiganders have turned to supplements this cold and flu season to bolster their body’s defenses.
While vitamins can help boost the immune system, too much of a good thing can have negative impacts, explained Sarah Hutchinson, a registered dietitian at Henry Ford Health.
Supplement toxicity is where you get too much of a vitamin or mineral, usually from supplements or artificial sources,” she said. “It’s becoming more of a problem with more readily available supplements.”
It’s very uncommon for individuals to intake a toxic amount of a given vitamin or mineral from food sources alone. But the addition of one or more supplements can lead to an intake of hundreds or thousands of percentage points beyond what’s recommended daily.
As an example, Hutchinson described a situation where a patient may be taking a daily multivitamin, plus a daily dose of a vitamin C supplement like Emergen-C, a zinc supplement, and an elderberry supplement that also includes zinc and vitamin C.
“They’re taking one product on top of another when they probably only need that multivitamin every day or every other day,” she said. “I heard a good metaphor recently: If you put more gas in a car, it’s not going to go faster. The same way with vitamins and minerals, just because we have more, it doesn’t mean your body will work better.”
Symptoms of getting too much of a vitamin or mineral can be general or nonspecific, like headache, stomachache, or heart flutter. More serious reactions can include blood clots and stroke-like symptoms, especially in individuals with liver or kidney problems.
Too much of one nutrient can also look like a deficiency of another, making it difficult to self-diagnose the issue. A blood test can help assess unhealthy levels; so can better evaluate food labels when determining what supplements to take.
The required nutrients listed on the typical nutrition facts label underwent a change in recent years to reflect nutrients many Americans do not sufficiently ingest. Labels still need to include calcium and iron, but Instead of requiring vitamins A and C, they now require vitamins D and potassium.
